The form included in this website should be downloaded and completed by the professional worker and signed by them. Notes of Guidance for completion of form are can be downloaded here.  The form should then be returned by post  to the Clerk of Taunton Aid in Sickness Fund c/o A.C.Mole & Sons, Stafford House ,Blackbrook Park Avenue,Taunton TA1 2PX. Additional information can be attached to the fully completed form, but applications will only be accepted on properly completed forms.

Cheques are only paid to referring agencies and they have responsibility for administering any money allocated and ensuring that it is used for the specified need applied for.
